Begin to fill out IRS Form 1040-EZ by entering your social-security number in the top, right-hand corner of the form and affixing the mailing label provided to you by the IRS in the "LABEL HERE" position. If you do not have a label or it includes incorrect information, enter your name and address in the fields provided.
Enter your wages, salary, tips, taxable interest, unemployment compensation and Alaska Permanent Fund dividends on lines 1, 2 and 3. Total your adjusted gross income on line 4.
Record your total standard deductions and personal exemptions on line 5. Include the adjustment specified on the back or second page of Form 1040-EZ if someone can claim you or your spouse as a dependent.
Subtract line 5 from line 4 and enter your taxable income on line 6.
Enter your federal tax withheld, from box 2 of your W-2 forms, on line 7.
Use the earned-income credit instructions, steps, worksheet and table from the IRS Form 1040EZ instruction booklet to figure your credit, and then fill out line 8(a).
Enter other credits, and total your payments, tax and any refund on lines 10, 11 and 12a.
Calculate any amount you owe on line 13. Make your check or money order for this amount payable to the "United States Treasury." Include your name, social-security number, phone number and "200_ Form 1040EZ" on the check (unless you intend to e-file).
Mail your signed and dated IRS Form 1040-EZ with your payment to the address shown for your state on page 2 of Form 1040-EZ. You also may e-file your return with the IRS website (see Resources below).
